Dentist and dental care in Spain: what public covers, kids and private insurance
Dental care in Spain is mostly private, but the public SNS portfolio includes basic oral-health services for everyone, extra coverage for children up to 14 and pregnant women, and specific risk groups. This guide explains what the public system covers, what remains private, and what to check before paying a clinic or buying dental insurance.
Public vs private dental
Most dental care in Spain is private: cleaning, fillings, crowns and orthodontics are paid (out of pocket or via a seguro dental).
The public system (SNS) covers a limited set of dental services: oral-health advice, treatment of acute dental conditions, extractions and minor oral surgery, early lesion detection, and additional services for children up to and including age 14, pregnant women and specific groups.
For routine treatment, choose a private clinic or a dental insurance plan — compare carencias, copagos and the network of clinics.
What the public system covers
- Anyone with the tarjeta sanitaria gets the common public dental portfolio: advice, acute care, pharmacological treatment when needed, extractions/minor oral surgery and early lesion checks.
- Children and young people: the common portfolio includes additional actions from birth up to 14 years inclusive; regional programmes may be called PADI or another name.
- Pregnant women have additional common coverage: oral-health advice, clinical examination and, when indicated, tartrectomy and preventive substances.

What to check before paying

- Keep invoices — some treatments may be deductible or covered by a mutua.
Getting dental care
- Check how your region implements the common SNS dental portfolio (adults, children up to 14, pregnancy and risk groups).
- For routine care, choose a private clinic or a seguro dental.
- Compare options on carencias, copagos and the cuadro of clinics — not just price.
- Always ask for a written presupuesto before starting treatment.
- For children, use the regional child dental programme if your community routes coverage through one.
- Keep the invoices for possible deductions or mutua reimbursement.
A real case: the clinic vanished and the bank returned the money
The iDental clinics closed, treatments were left unfinished and the loans stayed alive. The court ordered the finance companies to return the money. The Audiencia Provincial of Valladolid (First Section) decided claims by customers of the iDental chain which, in the court communication’s words, «desapareció sorpresivamente del mercado». The court reasoned from the link between the treatment contract and the consumer credit that paid for it: where the service was not provided, or provided defectively, the finance companies must return its price. In one patient’s case that was 1,713 euros, the full value of the financed treatment. The court refused, however, to make the lenders pay moral and material damages: «quedan fuera de dichas obligaciones las de indemnizar al consumidor por los eventuales daños materiales y morales derivados de la negligente actuación de Idental».
What was not obvious. When a clinic disappears, the instinct is to chase the clinic. But if the treatment was paid for with a linked credit, the money claim can be brought against whoever granted that credit. The rule still stands: art. 29.3 of Ley 16/2011 (read in the consolidated text on BOE) lets the consumer bring against the lender the same rights he has against the supplier — on two conditions: the goods or services were not delivered in whole or in part, or do not conform to the contract, and the consumer has already claimed against the supplier, in court or out of it, without obtaining satisfaction. The second, less pleasant lesson: getting your money back and being compensated are different things. The price of the treatment was returned; moral and material damage stayed with the clinic, not with the bank.
Outcome: the finance companies must return the price of defective or unperformed treatment — 1,713 euros in the case examined. Moral and material damages against the lenders were refused.
The limits of this case. It was decided on appeal in 2020 and binds only the parties. The rule it rests on is in force: art. 29.3 of Ley 16/2011 was read in the consolidated BOE text, and the «claim against the supplier first» condition is part of the rule itself, not our addition. The case concerns treatment paid for with a linked credit; if you paid from your own pocket, this route is not yours. A note on identifiers: the CGPJ communication prints the court and the section but neither the judgment number, nor the case number, nor an ECLI or ROJ, and the date of the judgment does not appear in its text either — it shows up only in the name of the attached file. The date shown here is therefore the date of the communication, and is labelled as such.
