Dentist and dental care in Spain: what public covers, kids and private insurance
Dental care in Spain is mostly private, but the public SNS portfolio includes basic oral-health services for everyone, extra coverage for children up to 14 and pregnant women, and specific risk groups. This guide explains what is public, what remains private, and what to check before paying a clinic or buying seguro dental.
Public vs private dental
Most dental care in Spain is private: cleaning, fillings, crowns and orthodontics are paid (out of pocket or via a seguro dental).

The public system (SNS) covers a limited common portfolio: oral-health advice, acute dental processes, extractions/minor oral surgery, early lesion detection, and extra actions for children up to 14, pregnant women and specific groups.
For routine treatment you choose a private clinic or a dental insurance — compare carencias, copagos and the cuadro of clinics.
What the public system covers
- Anyone with the tarjeta sanitaria gets the common public dental portfolio: advice, acute care, pharmacological treatment when needed, extractions/minor oral surgery and early lesion checks.
- Children and young people: the common portfolio includes additional actions from birth up to 14 years inclusive; regional programmes may be called PADI or another name.
- Pregnant women have additional common coverage: oral-health advice, clinical examination and, when indicated, tartrectomy and preventive substances.

Kids and pregnant women
- Dental acute processes, extractions and minor oral surgery are covered for people with a health card.
- Children up to 14 have additional common coverage; the regional programme name and appointment channel vary by autonomous community.
- Pregnant women have at least one dental visit pathway, preferably in the first trimester, with examination and indicated preventive actions.
What to check before paying
- Your tarjeta sanitaria for the public part.
- For private: ask for a written presupuesto (quote) before any treatment.
- For a seguro dental: the carencias (waiting periods), copagos per treatment and the network of clinics.
- Keep invoices — some treatments may be deductible or covered by a mutua.
Getting dental care
- Check how your region implements the common SNS dental portfolio (adults, children up to 14, pregnancy and risk groups).
- For routine care, choose a private clinic or a seguro dental.
- Compare options on carencias, copagos and the cuadro of clinics — not just price.
- Always ask for a written presupuesto before starting treatment.
- For children, use the regional child dental programme if your community routes coverage through one.
- Keep the invoices for possible deductions or mutua reimbursement.
Common mistakes
- Assuming the public system covers all dental — it does not; routine care is private.
- Starting treatment without a written presupuesto.
- Not checking the regional child dental programme or SNS appointment route.
- Buying a seguro dental with long carencias for what you need now.
A problem with a clinic
- If a private clinic overcharges, does poor work or won't honour the presupuesto, you can file a consumer claim (reclamación) — keep the quote and invoices.
